Job Description:

The Case Manager II is responsible for providing assessment and coordination of resources through the continuum of care, based on individual needs of the patient/family and resources available. This individual utilizes the Nursing and Case Management standards of practice and all current technology to conduct the assessment interview and then documents the information to accomplish the responsibilities of this role. Evaluation of the patient is based on prior level of care, current needs and continuum of care needs. The case manager II performs discharge planning and utilization management functions. This individual will be familiar with the uninsured population and be able to handle multiple tasks. The case manager provides leadership by managing patient care coordination through others using Standards of Practice, The Joint Commission, Title XXII, and the Nurse Practice Act and interpreting the Standards of Practice to other staff. The case manager demonstrates knowledge of case management, discharge planning, transfer coordination and utilization management functions. This individual will participate on department based committees, participate and promote Employee Opinion of Work activities, may function as a preceptor/competency evaluator and provide feedback to the manager. They will support Sutter Tracy Community Hospital Pillar Goals and Standards of Excellence by functioning as a six pillar team member.


Bachelor's of Science in Nursing degree preferred.

Licensure and Certifications

CA RN license and BLS for the healthcare provided issued by American Heart Association required.


Minimum of one-year experience as a registered nurse preferred.

Skills and Knowledge

The ability to get along with co-workers, peers, and patients. The ability to recognize potential hazards and follow appropriate precautions. The ability to understand and remember detailed instructions. The case manager requires knowledge of basic skills in the fundamentals of clinical nursing and pharmacology. Other skills required include but are not limited to: Joint Commission Standards. Required skills include Title 22 and other Local, State of California and Federal regulations. Required skills include California Board of Registered Nursing Scope of RN Practice. Patient Bill of Rights skills required. Patient Safety skills required. Environmental Safety skills required. Infection Control Management Principles skills required. Employee Health Practices and Regulations skills required. Organizational policies and procedures skills required. Fiscal Responsibilities skills required. Performance Improvement skills required. Five Principles of Patient Communication (AIDET) skills required. Computers skills required. OSHA skills required. COBRA Violations skills required. Ability to perform constant patient care activities that require fine motor skills and constant wrist action. Ability to be aware of surroundings and respond quickly to emergency and Unusual events - possibility of running. Ability to work in highly stressful environment. Ability to assist wtih possibly combative, disoriented patients. Ability to wear protective clothing.
