(Post-Acute / Long-Term Care) Position Overview We are seeking a
compassionate and skilled Occupational Therapist (OT) to join a
collaborative rehabilitation team dedicated to restoring
independence and improving quality of life for residents in a
post-acute and long-term care setting. This role offers the
opportunity to make a meaningful impact while working in a
supportive, resident-focused environment. Key Responsibilities -
Evaluate residents and develop individualized occupational therapy
treatment plans - Deliver therapeutic interventions to improve
functional mobility, ADLs, safety, and independence - Collaborate
closely with interdisciplinary teams including nursing and rehab
staff to achieve optimal outcomes - Monitor resident progress and
adjust treatment plans as needed - Maintain accurate and timely
documentation in compliance with state and federal regulations
Qualifications - Active California Occupational Therapist (OT)
license required - Strong clinical judgment and interpersonal
communication skills - Commitment to delivering high-quality,
patient-centered care - Skilled nursing or post-acute care
